The cost per team by age group is posted on the main page of the Fort Wayne Invitational Tournament webpage and also listed in the Permission to Host/Sanction Form. If you have lost or forgotten your team's log in ID number, please see the "Forgot your team ID number" which is located just under the Log In section on the front page of the FWI Tournament webpage. | Do I send a copy of my team roster or permission to travel? You will provide a copy of your team roster and a copy of the State's Permission to Travel when you check in your team at FWI Team Registration on Friday June 13, 2008 at the Host Hotel- Marriott Hotel. |

| How do I find out which field my team will play on? We do not have designated fields for each division. The fields are determined after we accept the teams, based on the number of teams per division and the available fields. Heffner Fields and Kreager Park have both small-sided and full-sized fields. Please continue to check the website for schedules and field assignments. |
| The application form does not look right on my screen (overlapping drop down boxes, long rows over the input fields, etc.) What is going on? First, you should let the ENTIRE form load BEFORE you scroll. It is a big form. In short, your computer does not have enough memory (RAM) available to render the form correctly. Here is the solution that will work in most cases: 1. Clear the cache (temporary files) in Internet Explorer (Usually Tools>Internet Options>Remove Temporary files 2. Shut down Internet Explorer 3. Restart the computer 4. Open JUST MS Internet Explorer. 5. Go to Web site, fill out form. In most cases, this will solve the problem.... |

| What hotel may I choose for my team to stay in? Your team is required to book hotel rooms using the hotels only listed under "Hotel" link of FWI Tournament webpage to participate in the Fort Wayne Invitational. Teams not staying at one or more of these properties, will not be considered for acceptance to the tournament. While you are free to select any hotel property you wish, you will be asked to VERIFY your rooming arrangements when you register your team on Friday night. The Fort Wayne Invitational Tournament Committee and FW USA Citadel FC organization assumes no liability by it's endorsement of a particular hotel. |

| When and where is Team Registration for 2008 Fort Wayne Invitational? Team registration is Friday, June 13, 2008 from 5:00pm to 9:00pm. Fort Wayne Marriott Hotel, 305 East Washington Center Road, Fort Wayne, Indiana 46825. Coaches or Team Representatives only. Refreshments will be served. |
| Does my team need to register on Friday night if our first game is at 10:00am Saturday? Yes- all team registrations are done on Friday night, June 13th. |
| I need duplicate forms. How do I get them? You will need your TEAM ID NUMBER to access the forms. When you log in, the system will tell you if your team has already been confirmed. There will be a link to the page that contains the duplicate forms. |
| What do I need for team registration night? Please bring the following to Friday night team registration June 13, 2008: 1. Original State Approved Roster and one copy of that roster to leave with the tournament 2. Validated Player Pass Cards..both rostered players and guest players 3. Validated Coaches Pass Card..one for each coach, if applicable. Maximum of three. 4. Tournament Liability Form which must be signed by all players and a parent or guardian 5. Travel Permit if outside of state of Indiana except for US Club Soccer 6. Guest Player Form US Club Soccer Requirements If registering under US Club Soccer affiliation, you will need to provide the following: 1. Approved Official Roster 2. Validated Player Pass for each player 3. Validated Coaches Pass for each coach..maximum of 3 coaches 4. Tournament Liability Release Form signed by all players and a parent or guardian You will also be asked to complete the FWI Team Contact Form with team contact information: cell phone and hotel WHILE YOU ARE ATTENDING THE TOURNAMENT in case we need to contact you. *Players over the age of 18 may sign their own forms |
| When is my team roster frozen? Your team roster is frozen at registration, Friday night, June 13th. |
